APHC27OPCC Theme: Stronger Together -- Humanity at the Heart of Palliative Care Palliative care is fundamentally about people: those receiving care, their families and carers, and the workforce and communities who support them. Yet care is increasingly delivered within complex systems that can fragment experiences and place pressure on relationships. The program explores how palliative care can remain grounded in humanity by strengthening connection, collaboration and compassion across systems, services and communities -- through interdisciplinary partnerships, meaningful community engagement, culturally responsive care and evidence-informed innovation. Program themes • Building bridges -- connecting systems, services, communities and cultures to improve care experiences • Diverse voices, shared purpose -- advancing equity, inclusion and care for underserved populations • Innovation with compassion -- practical clinical and service innovation that strengthens human connection • Growing together -- building capability, leadership and workforce sustainability from grassroots to government Program structure Tuesday 20 April is a dedicated pre-conference workshop day (optional, with a separate participation fee), followed by three full conference days through to Friday 23 April. Presentation streams Abstract submissions closed with more than 1,000 submissions received, assessed across five streams: • Systems and Solutions -- policy, funding and resourcing, integration, models of care, health services research, innovations, health systems strengthening • Communities and Culture -- culturally responsive care, community-led approaches, Indigenous perspectives, rural and remote, priority and underserved populations, cross-regional perspectives, compassionate communities, family/unpaid carers • Clinical Excellence across the Lifespan and across Contexts -- best practice across settings, complex care, interdisciplinary practice, paediatric/adolescent/young adult palliative care, palliative care in aged care and in chronic disease • Capacity and Leadership -- workforce development, education, wellbeing, leadership, volunteering, sustainable services, informal caregiver capacity building • Research and Innovations -- empirical research, methodology and measurement, implementation science, technology and AI, quality improvement, patient/public co-designed research An Emerging Work category also covers practice initiative projects, lived experience presentations and clinical trials not yet completed. Abstracts were blind peer reviewed against originality and innovation (30%), relevance and impact, alignment to the conference theme, and quality of content. Everyone who submitted is being notified of the outcome by early October 2026. If this is your first time submitting, the Abstract Mentoring Program supports presenters throughout the process.
