Hospital-to-Community Transition Liaison

A patient who's medically ready for discharge but still waiting on an NDIS plan review or a Home Care Package can end up stuck in an acute bed for weeks — a real, well-documented driver of hospital bed-blocking that has nothing to do with their actual clinical need. This liaison role works the seam between the acute system and the community-care approval processes, chasing paperwork, coordinating assessments and translating urgency across two systems that don't naturally talk to each other.
